Grants focus on areas relevant to our mission, including memory care, stroke recovery, grief support programs, services for the visually impaired and senior education. Grants also support building awareness of issues important to seniors in our community. These grants are supported by generous donations from residents at The Forum at Rancho San Antonio and surrounding communities.
Medical Care
- A grant to help establish a stroke recovery program at the Forum Health Care Center using LiteGait and the Interactive Metronome technologies. The grant allows for purchase of state-of-the-art equipment providing weight-bearing assistance for fall-free therapeutic activities, recovery support and staff training.
- Two grants to purchase equipment and to train staff for use of iN2L (It’s Never Too Late) new technology to improve quality of life for those in memory care at the Forum. Emerging technologies have shown potential to find a kinder and gentler path through the turbulent journey of such disease.
These grants introduce new technologies into the recently-opened Daffodil Ridge Memory Care Center at the Forum. The Forum Memory Care Center team has been nationally- recognized for its work in incorporating the iN2L technology into programs for memory care residents. This included recognition given to only six programs out of 3700 for the team’s innovative use of tablets, large screens and a content library to keep memory care residents engaged with their families during the pandemic.
- An additional grant was made to the Forum Health Care Center for fitness equipment for Assisted Living. This equipment will support strength, balance, energy and endurance for those in Assisted Living, helping to maintain independence and carry out everyday activities.

Social Services
- A grant was made to Community Services Agency, Mountain View, CA to support services provided to seniors. CSA is a social safety net agency providing vital social services to residents of Los Altos, Los Altos Hills and Mountain View. CSA provides critical support services that preserve and promote stability, self-reliance and dignity. https://www.csacares.org/
- A grant was made to AnewVista Community Services, focused on providing services for digital inclusion for at-risk seniors. ANVCS provides technology training programs for seniors in underserved areas, including in-person and remote technical training and support specialized for senior’s unique needs, in a safe and supportive community environment. ANVCS programs include regular access to technology experts, building confidence and enhancing seniors’ familiarity with essential everyday technology and building confidence. https://www.anvcs.org/
- A grant was made to Pacific Hearing Connection, a Los Altos-based nonprofit supporting those in our community who are not able to afford seeing a hearing specialist or buy hearing aids. Approximately 75% of their clients are over 65. Services include initial screening, wax removal, fitting hearing aids, counseling, and follow-up care. Our grant enables them to stock supplies and provide staff for screening and counseling at community healthcare events. https://www.pacifichearingconnection.org/
- Live Oak Adult Day Services seeks to provide socialization and cognitive stimulation to seniors in Santa Clara County and to support family caregivers in their efforts to maintain senior dependent loved ones in the family home. They developed an on line (Zoom) program that provides exercise, music and games for two hours daily. Our funding supports interns to continue to improve its content and delivery. This enables Live Oak to deliver the virtual program through calendar 2023. http://liveoakadultdaycare.org
- Grants were made in new collaboration with the Los Altos-Mountain View Community Foundation as part of the Forum Charitable Fund’s expanding outreach into our local communities. https://losaltoscf.org/
- A grant was made to KARA, a nonprofit providing grief support programs and services to families and individuals, including seniors. The grant, in partnership with LAMVCF, supports bereavement services to seniors. Services are offered in group settings and individually. KARA’s vision is to see people of all ages compassionately supported on their journey through grief so they can move toward renewed hope and meaning. KARA has received a second follow-on grant for continuing support of this program. https://kara-grief.org/
- A grant was also made in collaboration with LAMVCF to VISTA Center for the Blind and Visually Impaired to support services for seniors. VISTA provides vision loss rehabilitation through their Safe and Healthy Living Program, which has proven effective in helping visually impaired vulnerable seniors maintain their independence, live life with dignity and avoid early institutionalization. VISTA strives to empower individuals who are blind or visually impaired to embrace life to the fullest through evaluation, counseling, education, and training. VISTA has also received a second grant covering continuing services. www.vistacenter.org
Education & Research
- A grant to support the Forum's collaboration with Stanford researcher V.J. Periyakoil, MD, Director of AGE Research Center, studying the impact of isolation on seniors during the COVID-19 pandemic lockdown. http://aging.stanford.edu
- A grant to the Cupertino Library Foundation to support the development and delivery of a community speakers' series about senior issues.
- Several grants to the Fremont Union High School District Adult School to support a memoir writing class for seniors. http://fuhsd.org
